Compound Record Forms

QuickSCRIP offers the ability to generate Compounding Record Forms to maintain compliance with USP <795>. Information on USP <795> can be found here, see section labeled Compounding Documentation. These forms are intended to be generated with each dispensing, and stored for record-keeping purposes.

These documents include key information, such as:

  • Name, strength, and dosage of preparation
  • Master Formulation Record reference
  • Names and quantities of components
  • Sources, lot numbers, and expiration dates
  • assigned prescription number
  • preparation instructions

The Compound Record form works best if all relevant information in the Drug Compounding table has been filled out, as the form pulls lot numbers and ingredient information from this screen. For information on the QuickSCRIP Compounding system in general, click here.

Printing Compounding Record Forms

There are two methods in QuickSCRIP for generating Compound Record Form; QuickSCRIP can generate the forms automatically at the time of fill, or forms can be generated manually on demand.

To Print the form manually.

  1. Start in the Rx Processing Menu
  2. Choose 6 Edit an Rx.
  3. Search for a prescription and select it from the pick-list.
  4. Press & to pull up the Compound Table for this Rx.
  5. P option will print the Compound Record Form to your report printer.

Note: Printing from the edit screen will include prescription and customer information on the form. If you just want drug information, search for the drug in the Drug File Maint. and print from the compound table there.

To auto-print the form, you will need to set the Business Control File option.

  1. Start in the Pharmacy Main Menu
  2. Choose Option 3 Utilities
  3. Choose Option 3 Business Control File
  4. Select Screen 6 Additional Rx Custom Screen 3
  5. Line 39 Auto Print Compound Sheet
    • N tells it not to auto-print, and use the OLD FORMAT (does not include master form and preparation instructions). Forms must be printed manually (see below)
    • Y will automatically generate the Compounding Record Form on your report printer whenever a compound prescription is filled. Uses new format.
    • 3 will act the same as N, but uses new format for manual printing.

Master Instruction File

The Master Instruction File allows you to compile a database of instructions for multiple kinds of compounds in one location. Instructions are typed, associated with a code, and that code can then be attacked to all compounds that have similar mixing instructions. This saves you from having to type the same instructions for every compound. There is only one Master Instruction file; it is universal, and shared by all drugs in your system.

  1. Press I in any Compound Table, or && on any drug in the Drug File Maint. screen. This will pull up a text file that is separate from QuickSCRIP.
  2. Enter the desired directions as described below.
  3. Save and close the file.

Instruction Format

Instructions are entered into the Master File in the following format:

/CI=(Instruction Code)
Instructions line 1
Instructions line 2
Etc.
/CIEND

Instruction codes can be any name you want, up to 20 characters in length, with no spaces. New directions can be added after old ones, and in this way you build up a library of directions for drugs to reference.

Example:

/CI=TOPICCREAM
Weigh each ingredient.
Divide portions.
Mix Batch
Dispense into syringes
/CIEND

These master instructions can be referenced by specific drugs as discussed below.

Drug Specific Instruction File

Each Drug will have its own instruction file. This file will include Master Form Reference Codes, and

Storage Location, along with any compounding instructions unique to this drug.

  1. Start in the Drug File Maint screen.
  2. Search for a specific drug and select it from the pick-list.
  3. Press Shift + F11. This will pull up a text file that is separate from QuickSCRIP.
  4. Enter the desired directions as described below.
  5. Save and close the file.

Instruction Commands

The following commands can be entered into the drug instruction file. These commands are referenced by the Compound Record form when it is printed. If there are no commands attached to a drug, then the respective fields on the form will be blank.

  • /MFNUM= references the master compounding forms you keep in your records. No spaces.
  • /STORE= references the storage location and method of the compound. No spaces.
  • /INSTRUCT and /INSTRUCTEND all text between these commands print as instructions for the compound.
  • /CI= references instructions that exist in the Master Instruction File. Use the instruction code you created. Multiple codes can be stacked using a comma. This is ideal for compounds that have similar mixing directions, and can save you from typing the same directions repeatedly.

Note: Both the /INSTRUCT and /CI commands will print in the order they were entered into the file.

The following example shows what an instruction file might look like.

/MFNUM=TESTOST-OINTMENT
/STORE=BACKFRIDGE
/CI=TOPICCREAM,TESTOSTMIX
/INSTRUCT
BUNDLE SYRINGES INTO
4-PACK BOXES.
/INSTRUCTEND

In this example, the Compound Record would print TESTOST-OINTMENT for the name of the master form to reference, it would print BACKFRIDGE for the storage location, it would pull two sets of directs from the master instruction file (TOPICCREAM and TESTOSTMIX), and would also print additional directions to bundle the syringes into a 4-pack box.
